Fig. 8.6G to I A 50-year-old patient with posttraumatic severe equino-cavovarus deformity. Talus is round in the ankle mortise. Soft tissue distraction of the ankle joint is done in a nonconstrained manner. Here the shape of the articular surfaces guides and constrains the correction, given that distraction forces are applied in appropriate direction. The deformities are corrected sequentially, with forefoot supination being dealt with initially, thereafter forefoot adduction and cavus being corrected. Hindfoot inversion and equinus are corrected last with motors that stay perpendicular to the lever arm (line joining center of rotation of ankle to the point where the distraction forces are being applied).
